Repurposed medications have been investigated for potential activity against SARS-CoV-2, including ivermectin, an antiparasitic agent with reported antiviral and anti-inflammatory properties. Its efficacy has been evaluated in multiple clinical trials involving both treatment and prophylaxis of COVID-19.
A systematic review of randomized controlled trials (up to April 2021) included 24 studies with a total of 3,406 participants. Meta-analysis of 15 trials suggested a reduction in mortality in patients receiving ivermectin compared with control groups (risk ratio 0.38; 95% CI 0.19–0.73), with moderate-certainty evidence. Additional analyses indicated potential prophylactic benefit; however, evidence certainty varied across outcomes.
Secondary outcomes showed mixed results, with low to very low certainty evidence regarding clinical progression, need for mechanical ventilation, and recovery measures. Adverse events were generally uncommon and comparable between groups, though overall certainty of safety conclusions remained limited.
